Job Summary
Coordinate care for assigned clients with intellectual disabilities and psychiatric issues by monitoring medication, observing mental and social functioning, and maintaining contact with family and partners. Observe, signal, and align with colleagues and treatment providers to deliver optimal care within a multidisciplinary team. Support conversations between clients, physicians, and psychiatrists while contributing to restorative psychiatric care. Participate in multidisciplinary meetings and work within a specialized outpatient team during regular office hours.
