Job Summary
Conduct daily home care including basic hygiene, mobilization, and nutrition support. Administer delegated treatments such as medication, wound care, and injections. Monitor patient health status and relay relevant changes to medical staff. Assist with household tasks and maintain accurate nursing documentation. Serve as the primary contact for patients and family members. Collaborate with nurses, physicians, and therapists. Supervise nursing assistants within your scope of responsibility. Ensure compliance with hygiene, quality, and documentation standards per SGB XI and HPflBG. Work full-time or part-time with a pool of vehicles and a structured onboarding process. Located in Hamburg-Altona with excellent public transport access.
Required Qualifications
- Abgeschlossene Ausbildung als Gesundheits- und Pflegeassistent (GPA, 2-jährig nach HPflBG) oder Medizinischer Fachangestellter (MFA)
- Deutschkenntnisse mindestens B2
- Führerschein Klasse B
