Job Summary
Provide comprehensive post-acute care to adults in skilled nursing facilities across Morris County, New Jersey, working independently and collaboratively with physicians and advanced practice clinicians. Dedicate a minimum of two days per week to delivering high-quality patient care within the Post-Acute Care Division. Proficiency in EPIC EMR and Point Click Care systems is required for daily operations. Applicants must have at least two years of experience in skilled nursing facilities or with geriatric patients.
Required Qualifications
- Ability to work independently and collaboratively
- Engage with physicians and fellow advanced practice clinicians
- Ensure delivery of high-quality patient care to adults
- Available to dedicate a minimum of 2 days per week to providing post-acute care services
