Job Summary
Conduct geriatric assessments, manage polypharmacy, and diagnose age-related conditions for patients in a federally qualified community health center. Prescribe treatments, administer vaccinations, and explain procedures while maintaining strict confidentiality and following federal/state regulations. Collect and maintain patient records, refer specialists when necessary, and advise on diet, activity, and disease prevention. Attend monthly and quarterly medical staff meetings and provide 24-hour on-call service on a rotating weekly basis. Obtain and maintain admitting privileges to affiliated hospitals.
Required Qualifications
- MD/DO degree from an accredited medical school
- completion of an accredited residency program
- completion of a Fellowship in Geriatrics
- Valid NY State license
- CDS
- DEA
- BLS
- Board Certified
