Job Summary
Conduct home visits, perform comprehensive assessments, and collaborate with an interdisciplinary team to develop person-centered care plans for older adults and individuals with disabilities. Manage constant change while maintaining excellent organizational skills, and utilize data entry and reporting tools to track services. Requires a Bachelor's degree or equivalent experience, a reliable vehicle with a clean driving record, and the ability to work flexible schedules including alternate start times and potential telecommuting. Springwell supports aging-in-place initiatives with a focus on dignity and independence.
Required Qualifications
- Bachelor's Degree or higher
- Associate's Degree and at least 3 years' experience working with elders or providing case management services
- completed at least 50% of coursework required for a Bachelor's Degree and agrees to complete the remaining coursework within a 3-year period
- A strong desire to work with seniors and individuals with disabilities
- Excellent organizational skills and the ability to manage constant change with a positive attitude
- Experience working with computers, entering data, and running reports
