Job Summary
Perform medical examinations, evaluations, assessments, diagnoses, and treatments for long-term chronic care patients in post-acute inpatient and outpatient settings. Prescribe pharmaceuticals and treatment regimens, refer patients to specialists, and direct clinical team activities to meet individual patient needs. Document encounters daily in the electronic medical record system and ensure timely charge submission. Participate in overnight, weekend, and holiday calls as required. Train and supervise medical students, residents, and fellows while contributing to quality improvement, medical staff meetings, and health promotion programs. Maintain unrestricted Massachusetts medical license and ABIM certification. On-site role at Hebrew Rehabilitation Center.
Required Qualifications
- Awarded degree of MD or DO
- ABIM recognized internal medicine or family practice residency (3 years minimum)
- Successful completion of ACGME accredited geriatric medicine fellowship program
- Valid and unrestricted Massachusetts Medical License
- American Board Certification in Internal Medicine or Family Medicine currently valid or renewed (if needed) prior to expiration
- Federal DEA Certification
- CPR, BLS, ACLS as indicated by specific practice location
- Experience, gained in training and/or in practice, caring for seniors in long term chronic care, community or home settings
- Experience delivering effective medical care within an inpatient environment
- Ability to maintain quality, safety, and/or infection control standards
- Knowledge of community medical diagnostic and patient care services in area of medical expertise
- Experience working both independently and in a team environment
- Knowledge of current principles, methods and procedures for the delivery of medical evaluation, diagnosis and treatment in area of expertise
- Ability to supervise, advise and train clinical professionals and/or students in area of expertise
- Participate in overnight, weekend and holiday call as appropriate for area of practice
Desired Qualifications
- ABIM added certification in geriatric medicine
- Teaching responsibilities in connection with the Harvard Medical School Fellowship in Geriatric Medicine
