Geriatric Primary Care Physician | Peak Primary Care | Thornton

Conduct outpatient geriatric primary care for approximately 18 patients daily in an independent private practice setting. Provide APP supervision as required, adhering to a 1:6 call schedule. Utilize Allscripts EHR for documentation and care coordination. Board eligible or certified geriatricians with fellowship training or subspecialty interests in geriatrics are qualified for this 1.0 FTE role. Join a team of eight providers supported by PHPprime services including care coordination, risk adjustment coding education, and population health management.
