Job Summary
Conduct comprehensive evaluations of patients' physical capabilities to assess specific needs and challenges, ensuring a tailored approach to treatment. Design and implement personalized treatment plans utilizing therapeutic exercises, manual therapy, and advanced modalities to promote recovery and independence. Monitor and document each patient's progress meticulously, making necessary adjustments to treatment protocols to ensure optimal therapeutic outcomes. Collaborate effectively with a multidisciplinary team, including physicians, occupational therapists, and speech therapists, to coordinate integrated care strategies. Teach patients and their families about effective therapeutic techniques, fostering understanding of home exercise programs and injury prevention measures. Develop and execute fall prevention initiatives and safety protocols aimed at minimizing injury risks within assisted living environments. Engage in continuous professional development through educational opportunities to stay abreast of advancements in physical therapy practices and methodologies. Uphold the highest ethical standards and patient confidentiality while consistently delivering quality care.
Required Qualifications
- A graduate from an accredited college or university with a Bachelor's degree or higher in Physical Therapy
- Possession of an active state license to practice as a Physical Therapist
- Exceptional interpersonal and communication skills for effective interaction with both patients and healthcare colleagues
- Familiarity with electronic medical records (EMRs) and proficiency in documentation processes
- A strong understanding of evidence-based practices in physical therapy and a commitment to integrating the latest techniques into patient care
- Monday to Friday, from 9 AM to 5 PM
- Primarily onsite
