Job Summary
Conduct comprehensive evaluations of patients' physical capacities using evidence-based assessment tools and develop individualized treatment plans focused on geriatric rehabilitation goals. Provide therapeutic services including exercises, manual therapy, and modalities while monitoring progress through detailed reporting to adjust plans for enhanced recovery outcomes. Collaborate actively with a multidisciplinary team of healthcare professionals to deliver coordinated care and educate patients and families on injury prevention and home care techniques. Implement fall prevention programs to create safe environments and maintain patient confidentiality in compliance with healthcare regulations. This full-time onsite role at our North Houston Rehabilitation Center supports our mission of empowering elderly clients to age successfully in their own homes.
Required Qualifications
- A Bachelor's degree in Physical Therapy from an accredited program
- Current and valid State licensure as a Physical Therapist
- Strong interpersonal skills to effectively communicate with patients, families, and the healthcare team
- Proficiency in electronic medical records (EMRs) and relevant documentation processes
- Commitment to patient-centered care and improving patients' quality of life through medical intervention and education
- Ability to work independently while being an effective collaborator within a team setting
Desired Qualifications
- At least one year of experience in geriatric therapy
- Familiarity with innovative therapeutic techniques and a commitment to evidence-based practice
