Job Summary
Conduct comprehensive geriatric physical therapy evaluations and deliver personalized treatment plans using therapeutic exercises, manual therapy, and modern modalities. Monitor patient progress, document care in electronic medical records, and adjust interventions to ensure optimal recovery. Collaborate with physicians and specialists on multidisciplinary care while educating patients and families on home exercise regimens and preventative strategies. Implement fall prevention programs and safety assessments within assisted living settings, adhering to ethical standards and confidentiality. Maintain licensure and pursue continuous education to stay abreast of evidence-based practices.
Required Qualifications
- A Bachelor's degree in Physical Therapy from an accredited institution
- Active state licensure as a Physical Therapist
- Strong interpersonal and communication skills
- Familiarity with electronic medical records (EMRs) and efficient documentation processes
Desired Qualifications
- Doctorate or Master's degree in Physical Therapy
- At least one year of experience in geriatric therapy
- Demonstrated ability to work independently, as well as collaboratively, within a healthcare team
