Job Summary
Assess, diagnose, treat, and manage acute and chronic conditions in older adults across office, home, and senior living settings. Lead Medicare Annual Wellness Visits, prescribe and reconcile medications, and provide care management for high-risk and recently hospitalized patients. Guide advance care planning and goals-of-care conversations while conducting home and facility visits. Maintain prescriptive authority and DEA registration to support comprehensive geriatric care for approximately 1,600 Medicare patients with complex needs. Required Qualifications
- MSN or DNP from an accredited Nurse Practitioner program
- Active Massachusetts NP/APRN license, with the ability to obtain New Hampshire licensure
- Board certification as an Adult-Gerontology, Gerontological, or Family NP (ANCC or AANP)
- Prescriptive authority and DEA registration, or eligibility to obtain
- Extensive experience caring for geriatric patients
- Valid driver's license and reliable transportation for home and facility visits
- Must be able to obtain New Hampshire licensure
