Job Summary
Geriatric Medicine Physician Faculty role leading and teaching within a population health-focused, age-friendly care model. Responsibilities include delivering longitudinal care to older adults with complex medical, cognitive, and social needs; leading population health initiatives; embedding the IHI 4Ms framework (What Matters, Medication, Mentation, Mobility) into clinical practice and education; developing innovative care models for dementia and cognitive decline; partnering with caregivers; mentoring fellows and residents; serving as core faculty in the geriatric fellowship; collaborating across disciplines in an integrated, team-based care model; contributing to quality improvement, curriculum design, and scholarly work. Ideal candidate is board-certified in geriatric medicine, passionate about transforming aging care, and committed to teaching and leadership within a supportive, purpose-driven environment.
Required Qualifications
- Board-certified in geriatric medicine (MD or DO)
- Experience or deep interest in population health, dementia care, and team-based care models
- Commitment to teaching, mentorship, and innovation
