Job Summary
Board-certified geriatric medicine physician faculty leader sought to deliver comprehensive, longitudinal care to older adults with complex medical and social needs. Lead population health initiatives, build and advance an Age-Friendly Health System incorporating the IHI 4Ms (What Matters, Medication, Mentation, Mobility), lead care models for dementia and cognitive decline, mentor fellows and residents, teach in a geriatric fellowship, and collaborate in a fully integrated, team-based care model while contributing to quality improvement, curriculum design, and scholarly work.
Required Qualifications
- Board-certified in geriatric medicine (M.D. or D.O.)
