Job Summary
Board-certified geriatric medicine physician faculty lead position focused on aging care transformation. Deliver comprehensive, longitudinal care to older adults with complex medical, cognitive, and social needs; lead population health initiatives and design an Age-Friendly Health System integrating the IHI 4Ms framework (What Matters, Medication, Mentation, Mobility). Teach and mentor fellows, residents, and interprofessional learners; serve as core faculty in the geriatric fellowship with leadership opportunities; collaborate in an integrated, team-based care model and contribute to quality improvement, curriculum design, and scholarly work. The role emphasizes education, mentorship, and shaping care models for dementia, cognitive decline, and caregiver engagement in a mission-driven, community-health setting in Scranton, PA.
Required Qualifications
- Board-certified in geriatric medicine (M.D. or D.O.)
