Job Summary
Geriatric Medicine Physician Faculty leading aging-care initiatives in an academic clinical setting. You will deliver comprehensive longitudinal care to older adults with complex medical, cognitive, and social needs, serve as population health leader to improve outcomes for high-risk aging populations, and help build an Age-Friendly Health System by embedding the IHI 4Ms framework (What Matters, Medication, Mentation, Mobility) into clinical practice and education. The role includes leading innovative care models for patients living with dementia and cognitive decline, partnering with caregivers as essential members of the care team, teaching and mentoring fellows and residents, serving as core faculty in the geriatric fellowship program with leadership opportunities, collaborating across disciplines in a fully integrated, team-based care model, and contributing to quality improvement, curriculum design, and scholarly work. Location: Scranton, PA. The position emphasizes mission-driven, whole-person care, health equity, and a collaborative, purpose-driven culture.
Required Qualifications
- Board-certified in geriatric medicine (M.D. or D.O.)
- Experience or strong interest in population health, dementia care, and team-based models
- Committed to teaching, mentorship, and innovation
