Job Summary
Lead as a Geriatric Medicine Physician Faculty: deliver comprehensive longitudinal care for older adults with complex medical, cognitive, and social needs; serve as a population health leader to improve outcomes for high-risk aging populations; build and advance an Age-Friendly Health System embedding the IHI 4Ms framework (What Matters, Medication, Mentation, Mobility); lead innovative care models for dementia and cognitive decline; mentor fellows, residents, and interprofessional learners; contribute to quality improvement, curriculum design, and scholarly work; collaborate across disciplines in an integrated team-based care model; participate in teaching, mentorship, and leadership within the geriatric fellowship program.
Required Qualifications
- Board-certified in geriatric medicine (M.D. or D.O.)
