Job Summary
Board-certified geriatric medicine physician faculty in a leadership role responsible for delivering comprehensive, longitudinal care to older adults with complex medical, cognitive, and social needs; driving population health initiatives; building an Age-Friendly Health System aligned with the IHI 4Ms (What Matters, Medication, Mentation, Mobility); leading innovative care models for dementia and cognitive decline; mentoring fellows/residents and contributing to curriculum design, quality improvement, and scholarly work; collaborating across disciplines in an integrated, team-based care model and supporting caregivers and families.
Required Qualifications
- Board-certified in geriatric medicine (M.D. or D.O.)
- Experience or deep interest in population health, dementia care, and team-based models
- Commitment to teaching, mentorship, and innovation
