Job Summary
Lead clinical faculty role in geriatric medicine focused on aging care transformation, teaching fellows and residents, and building an Age-Friendly Health System using the IHI 4Ms framework. Responsibilities include delivering longitudinal care to older adults with complex medical, cognitive, and social needs; serving as population health leader; designing and embedding team-based care models; leading care for patients with dementia and cognitive decline; partnering with caregivers; mentoring fellows and residents; and contributing to curriculum design and scholarly work.
Required Qualifications
- Board-certified in geriatric medicine (M.D. or D.O.)
