Job Summary
Geriatric Medicine Physician Faculty leading population health initiatives and education. Role includes delivering longitudinal care to older adults with complex medical needs, leading the development of an Age-Friendly Health System using the IHI 4Ms framework (What Matters, Medication, Mentation, Mobility), and serving as core faculty in the geriatric fellowship with opportunities for leadership. Responsibilities span clinical care, population health leadership, curriculum design, quality improvement, and mentoring fellows, residents, and interprofessional learners within a fully integrated, team-based care model. The position emphasizes transforming aging care, teaching the next generation of clinicians, and advancing patient- and family-centered models of care; location in Scranton, PA.
Required Qualifications
- Board-certified in geriatric medicine (M.D. or D.O.)
- Experience or strong interest in population health, dementia care, and team-based care models
- Teaching, mentorship, and leadership interest or experience
