Job Summary
Conduct comprehensive assessments of geriatric patients to develop tailored treatment plans addressing mobility, strength, and wellness goals. Deliver compassionate therapy sessions utilizing therapeutic exercises, manual therapy, and various modalities to enhance recovery. Continuously monitor patient progress, documenting findings accurately and modifying plans to optimize outcomes while designing fall prevention initiatives. Engage in collaborative care with physicians, occupational therapists, and speech therapists to create holistic approaches. Educate patients and families on therapeutic exercises and home care strategies. Stay abreast of latest advancements through ongoing education and adhere to ethical standards ensuring patient confidentiality. Work Monday through Friday, 9 am to 5 pm, within our outpatient clinic in Greeley, Colorado.
Required Qualifications
- A graduate from an accredited institution with a Doctorate, Master's, or Bachelor's degree in Physical Therapy
- Active State licensure as a Physical Therapist
- Strong interpersonal communication skills
- Proficiency in utilizing electronic medical records (EMRs) and documentation systems related to patient care
- A solid understanding of evidence-based therapy techniques
- A patient-centered approach that prioritizes improving patient independence and enhancing their quality of life
- Demonstrated ability to thrive both independently and as part of a multidisciplinary team
- Experience working with geriatric populations
