Genesis Care Specialist

Maintain roles and responsibilities of a Genesis/HIV care manager while providing support to other care managers. Oversee quality improvement and care quality initiatives for the site, ensuring program responsiveness to Ryan White grant requirements. Attend case conference meetings to ensure timely, consistent service delivery to HIV+ patients and facilitate peer coverage during absences. Assist in supervising program peers and care managers when applicable, orient social work interns and staff to the site, and provide intervention, support, and problem-solving for staff and patient issues. Develop community-based resources and build relationships with collateral agencies. This part-time role in Coram, NY, offers an hourly rate of $35.24 - $44.05 for a Master's degree in social work.
