Generalist Assessor - Sioux Falls

Conduct initial and ongoing assessments using approved tools to evaluate functional status, service needs, supports, and risks. Complete all required documentation accurately and on time while communicating findings to designated parties. Coordinate with members, caregivers, case managers, MCOs, and agency staff as needed to develop personalized care plans. Adhere to CareStar policies, confidentiality rules, and reporting expectations while participating in training, quality assurance, and calibration activities. Requires a Bachelor's degree or four years of experience in intellectual, developmental, or physical disabilities; valid driver's license and reliable transportation for field visits.
