Generalist Assessor - Sioux Falls and Surrounding Counties

Conduct initial and ongoing assessments using approved tools to evaluate functional status, service needs, supports, and risks. Complete all required documentation accurately and on time while communicating findings and follow-up needs to designated parties. Coordinate with members, caregivers, case managers, MCOs, and agency staff as needed. Adhere to CareStar Rules, policies, and confidentiality standards regarding patient information. Participate in training, quality assurance, and calibration activities. Requires a Bachelor's degree or four years of experience in intellectual, developmental, and/or physical disabilities.
