Job Summary
Develop patient-centered transition plans by integrating evidence-based clinical guidelines, preventative protocols, and metrics to ensure quality and efficiency in post-acute care delivery. Represent Enhabit's home health branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians, and physician groups to drive referral source satisfaction. Maintain a valid state driver's license, automobile liability insurance, and dependable transportation in good working condition to safely drive in all weather conditions. This role supports Enhabit's mission as a national leader in home-based care, fostering an innovative, collaborative culture that expands possibilities for patient care in the home.
Required Qualifications
- Must be a graduate of an approved school of nursing or therapy
- Must be licensed in the state where they currently practice
- Must have two (2) years of demonstrated field experience
- Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice
- Must possess a valid state driver license
- Must maintain automobile liability insurance as required by law
- Must maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
- A Registered Nurse RN or Physical Therapist PT is required
Desired Qualifications
- Three (3) years of field experience is preferred
- Previous experience in home health or healthcare sales is preferred
