FYI Case Manager

Job Summary
Create self-sufficiency plans for foster care youth and alumni covering education, vocational training, and living skills. Review and update individualized service plans, case notes, and financial distributions while assisting participants with community resources like housing and program enrollments. Develop professional relationships with agencies for education, job training, and child care to coordinate services. Deliver independent living skills curriculum and travel throughout a multi-county region to provide services and enhance program delivery. Liaise with community agencies and TDFPS representatives, hosting quarterly meetings and promoting the FYI program. Maintain compliance with TWC grant documentation and policies.
Required Qualifications
- Bachelor's Degree in a related field
- Minimum 1-year prior related casework experience
- Proficient knowledge of and/or work experience using community resources
- Ability to speak, read and write English
- Ability to speak, read, and write Spanish
- Prior related experience in delivering services to youth and/or young adults
- Experience in the delivery of vocational or other self-sufficiency services
- Ability to drive assigned vehicle(s) or personal vehicle, with appropriate state license, following all laws applicable
- Must provide proof of liability insurance
- Must be eligible to be insured under Buckner's insurance policy
- Must be age 21 or older to drive on behalf of Buckner
- Ability to use up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force continuously to move objects and/or people
- Ability to walk, stand and sit, sometimes for prolonged periods of time
