Job Summary
Act as a liaison between long-term care homes, retirement communities, and CareRx Pharmacy to ensure optimal resident care and clinical support. Travel to assigned LTC homes and retirement communities to complete medication reviews, conduct on-site audits, and ensure regular visits. Build and maintain strong partnerships with community leadership teams while providing education to residents and staff on health topics, the MedsCheck program, and protocols such as influenza prevention. Collaborate with physicians, nursing staff, and other health providers to maintain continuity of care in alignment with site policies, internal standards, and provincial regulations. Complete all required documentation, billing processes, and participate in pharmacy-related transition processes including new client and eMAR transitions. Support CareRx Pharmacy policies and procedures, attend clinical team meetings, and contribute to the development of medication audit follow-up improvement plans.
Required Qualifications
- Insured and currently licensed in good standing with the Alberta College of Pharmacy (ACP)
- Complete understanding of provincial regulations governing LTC homes and Retirement Communities
- Must be familiar with the provincial standards of practice established for LTC pharmacies
- Must possess a valid driver's license
- Must be able to interact with persons of varying levels of professional expertise
- Must demonstrate initiative and creativity
- Self-directed requiring minimal supervision and able to work independently
- Motivated to achieve results that reflect a high standard of quality
- Dependable and responsible in the execution of job duties
- Adaptable to changes within internal work structures
- Strong presentation and teaching skills
