Job Summary
Perform initial, periodic, and change of condition functional screens for Family Care, PACE, and Partnership Program members. Provide ongoing training to certified staff, develop LTCFS training programs, and assist new Care Management staff in obtaining certification. Track timely screen completion in the computer system and participate in team meetings. Offer intensive oversight of all LTCFS activities for a minimum of six months to ensure screener competence. Requires a valid driver's license, proof of insurance, and regular local travel. Monday to Friday, 8:00 AM – 4:30 PM. Includes a $5,000 sign-on bonus. Bachelor's degree in Social Work, Psychology, or Human Services required, with current Long-term Care Functional Screener certification and 18 months of social service experience.
Required Qualifications
- Bachelor's Degree in Social Work, Psychology, Human Services or related field
- Current Long-term Care Functional Screener certification
- 18 months of experience in a social service capacity
- Master's degree in Social Work, Psychology, or Human Services
- one year experience with older adults or persons with physical and or intellectual disabilities in a social service capacity
