Job Summary
Receive telephone calls from the Triage Nurse during assigned shifts and respond within 20 minutes. Make home visits as directed, perform patient assessments to derive nursing diagnoses, and assess needs for hospice and home health aide services. Provide pain management strategies, patient/family teaching, and emotional support to foster independence. Plan nursing interventions in collaboration with the interdisciplinary team to resolve current or potential problems. Comply with Hospice of Huntington, Inc. policies, privacy regulations, and the hospice philosophy. Maintain a valid driver's license and reliable transportation for home visits.
Required Qualifications
- Weekend On-Call availability
- Available to receive telephone calls from the Triage Nurse
- Ability to respond to all calls within 20 minutes
- Ability to make home visits as directed
- Ability to perform assessment of patients and derive nursing diagnosis based on assessment data
- Ability to assess the need for hospice aide services
- Ability to assess and provide pain management strategies to assist with patient comfort
- Ability to plan nursing interventions in collaboration with the interdisciplinary team
- Ability to provide teaching to patients/families
- Ability to provide emotional support to patients/families
- Philosophy compatible with the hospice concept
- Graduate of an accredited professional school of nursing
- Licensed registered nurse in West Virginia and Ohio
- Minimum of 2 years registered nursing experience
- Proof of liability insurance
- Current driver's license
- Automobile insurance
- Reliable transportation
- Successful completion of a background check
- Successful completion of a drug screening
- Possession of a valid driver's license
