Job Summary
Provide individual and family counseling to patients in skilled nursing and long-term care settings while coordinating care and consulting with facility teams. Adhere to state and federal clinical guidelines and PGS protocols to educate staff and support holistic, patient-centered care. Maintain clear, timely documentation for all interactions. Work autonomously within a Monday–Friday day-shift schedule of 32 weekly hours, with no nights, weekends, or on-call rotations. This role supports the mission of expanding access to quality mental healthcare for seniors through collaborative, empathetic care in a transparent culture.
Required Qualifications
- State licensure as a Psychologist, Licensed Clinical Professional Counselor, or Clinical Social Worker (MD, DC, VA, or PA required)
- Monday–Friday availability during normal business hours
- Ability to stand for 20 minutes and lift up to 15 pounds
