Job Summary
Provide individual and family counseling to patients in skilled nursing and long-term care settings. Coordinate care by consulting with facility teams including administrators, directors of nursing, and social services. Adhere to state and federal clinical guidelines while maintaining clear, timely documentation. Educate staff to support holistic, patient-centered care within an integrated leadership environment. Work autonomously with access to robust clinical and administrative support. The role offers flexible Monday–Friday day-shift hours with no nights, weekends, or on-call schedules, totaling 32 weekly hours.
Required Qualifications
- State licensure as a Psychologist, Licensed Clinical Professional Counselor, or Clinical Social Worker (MD, DC, VA, or PA required)
- Monday–Friday availability during normal business hours
- Ability to stand for 20 minutes and lift up to 15 pounds
