Job Summary
Provide individual and family counseling to patients in skilled nursing and long-term care settings while coordinating care with facility teams. Adhere to state and federal clinical guidelines and PGS protocols to ensure holistic, patient-centered care. Maintain clear, timely documentation and educate staff on clinical best practices. Work autonomously with access to robust clinical and administrative support. No nights, weekends, or on-call schedules; flexible Monday–Friday day-shifts with 32 weekly hours. State licensure as a Psychologist, Licensed Clinical Professional Counselor, or Clinical Social Worker is required.
Required Qualifications
- State licensure as a Psychologist, Licensed Clinical Professional Counselor, or Clinical Social Worker (MD, DC, VA, or PA required)
- Monday–Friday availability during normal business hours
- Ability to stand for 20 minutes and lift up to 15 pounds
