Job Summary

Develop individualized service and housing plans that reflect each guest's strengths, goals, and needs using Housing First, trauma-informed, and harm reduction principles. Deliver Section 13 and/or Section 17 MaineCare Targeted Case Management services for eligible participants while assisting guests in obtaining permanent housing, income, benefits, healthcare, and community supports. Advocate for guests by building collaborative relationships with landlords, housing providers, healthcare organizations, and community partners. Maintain timely documentation in HMIS and electronic health records, then participate as an active member of a multidisciplinary team dedicated to ending homelessness in Central Maine.

Required Qualifications

  • Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Behavioral Health, or a related field or current MHRT-C certification
  • Valid driver's license and reliable transportation
  • Strong communication, organizational, and documentation skills
  • Ability to work independently while collaborating effectively within a multidisciplinary team

Desired Qualifications

  • Experience working with individuals experiencing homelessness, mental illness, substance use disorders, or complex trauma
  • Knowledge of Housing First, trauma-informed care, motivational interviewing, or harm reduction practices
  • Familiarity with MaineCare Targeted Case Management, HMIS, or electronic health records
