Job Summary
Establish and maintain relationships of trust with service users to build engagement while helping them set, prioritize, and achieve goals. Carry out risk assessments and assessments to determine urgency of needs, ensuring safety and improving support quality. Work flexibly with service users in their homes and communities, assisting with domestic tasks, personal care, medication management, and navigating benefits and education systems. Accompany service users to appointments and collaborate with professionals from other agencies for referrals. Make timely, accurate records using Together computer-based systems and write clear reports. Maintain professional boundaries and adhere to Together policies on confidentiality, GDPR, and equalities.
Required Qualifications
- Experience in supporting vulnerable individuals gained from paid work and/or voluntary and/or life experience
- Able to travel
- Able to work a varying pattern of shifts
- DBS clearance
- Minimum age 18 plus
Desired Qualifications
- Lived experience of mental distress
- NVQ Level II in Health and Social Care
