Job Summary
Conduct screenings, evaluations, re-evaluations, discharge planning, and revise established plans of care under physician supervision. Maintain active state licensure and complete all required certifications and screenings. Apply current knowledge of Functional Pathways policies and clinical programs to therapy disciplines. Deliver meaningful therapy services in a resident-centered environment within The Cedars of Chapel Hill Life Plan Community, supporting older adults through exceptional wellness-focused care and personalized services.
Required Qualifications
- Master's degree in Speech Language Pathology from an accredited program
- Certification and licensure requirements as established by state and national board associations
- Active state license
- Excellent organizational and time management skills
- Excellent written and verbal communication skills
- Possess current working knowledge of Functional Pathways policies and procedures, clinical programs with application skills to therapy disciplines
- Completion of all required screenings, certifications, and licensure
