Full-Time RN Case Manager (RN) | Symbii Home Health & Hospice | Soda Springs/Montpelier, ID

Conduct comprehensive initial assessments of patients and families to determine home care needs, regularly re-evaluate nursing requirements, and initiate revised plans of care based on changing status. Develop therapeutic, preventive, and rehabilitative nursing actions while administering prescribed medications and treatments. Coordinate patient care for assigned caseloads as a Case Manager when directed, ensuring all documentation adheres to organizational standards and procedures. Communicate with physicians regarding patient conditions and community health resources to maintain coordinated care plans. Participate in on-call duties as defined by policy and arrange necessary equipment and services.
