Job Summary
Conduct thorough examinations and evaluations of patients with functional limitations, developing personalized treatment plans in collaboration with patients, families, and interdisciplinary team members. Implement and monitor treatment interventions while maintaining accurate documentation of progress notes and charges for services. Actively supervise and mentor support staff, including physical therapy aides and students, and coordinate care with healthcare providers to ensure smooth transitions between levels of care. Engage in community outreach, patient education, and program development activities to support continuous improvement initiatives. This role involves working onsite at our Woodbridge facility serving diverse populations including children, adults, and seniors.
Required Qualifications
- A Bachelor's Degree in Physical Therapy
- A valid state license to practice Physical Therapy
- Basic Life Support (BLS) certification
- Exceptional verbal and written communication skills
- Proficiency in word processing and electronic health record (EHR) systems
Desired Qualifications
- Experience in a rehabilitation setting
- Knowledge of current therapeutic trends and evidence-based practices
