Job Summary
Conduct thorough evaluations of patients in a home care environment, assessing their needs and physical capabilities. Develop and implement individualized treatment plans incorporating manual therapy, exercise, and assistive technology to improve patient outcomes. Utilize modern therapy modalities to enhance rehabilitation efforts and document treatments, progress, and outcomes diligently. Educate patients, caregivers, and families on self-care strategies and assistive device usage. Collaborate with interdisciplinary teams to provide holistic care while participating in professional development through workshops and continuing education. Required Qualifications
- Doctorate degree in Physical Therapy (DPT)
- Valid New Jersey state license in Physical Therapy
