Job Summary
Conduct thorough evaluations to assess patients' physical abilities and develop personalized treatment plans based on findings, medical history, and therapy goals. Engage and educate patients and their families about evaluation results, treatment options, and expected outcomes. Implement various therapeutic interventions, leveraging clinical judgement to adapt protocols based on patient progress and evolving healthcare standards. Collaborate with other healthcare providers to discuss patient progress and modify care plans when required. Document all treatment-related activities meticulously and promptly, including patient evaluations, daily progress notes, and discharge summaries, utilizing the electronic health record (EHR) system. Maintain an organized and efficient workflow while meeting established productivity benchmarks. This full-time, onsite position serves diverse populations including post-operative recovery, chronic pain, athletic injuries, and neurological conditions.
Required Qualifications
- A Bachelor's Degree in Physical Therapy from an accredited institution
- A valid state license to practice physical therapy in Connecticut
- Certification in Basic Life Support (BLS) from the American Heart Association
Desired Qualifications
- A Doctorate in Physical Therapy (DPT)
- At least 2 years of hands-on physical therapy experience
- Experience within a rehabilitation facility
