Job Summary
Provide full-time geriatric/post-acute clinical care across 1-2 community sites in metro Atlanta, focusing on post-acute rehab, sub-acute care, and long-term care for an older adult patient panel. Collaborate within a growing multidisciplinary team of physicians and APPs to manage admits, transitions of care, discharge planning, and long-term primary care, with an emphasis on improving outcomes and continuity of care. Faculty appointment through Emory School of Medicine with opportunities to teach geriatrics fellows, FM/IM residents, and medical students; participate in research and quality-improvement initiatives; optional leadership-development pathway and Certified Medical Director role. Monday-Friday work with potential flexibility and a shared call rotation, offering compensated salary, sign-on/retention bonuses, and comprehensive benefits.
Required Qualifications
- Board Certified/Board Eligible in Geriatrics preferred
- FM or IM with experience in complex care management and multidisciplinary collaboration
