Job Summary
Conduct health assessments, develop personalized care plans, and evaluate patient conditions for a panel of geriatric patients residing in long-term care facilities and private homes. Perform minor procedures and wound care as required, educate families on health management, and coordinate with nurse practitioners and other healthcare professionals. Adjust treatment plans based on patient responses and utilize a mobile-based EHR with AI functionality to streamline administrative tasks. On-call rotation is phone triage only, typically one night per month. This role supports an organization dedicated to delivering high-quality primary care without the need for patients to travel to an office.
