Full-Time Physician - Geriatric Primary Care

Conduct health assessments, develop personalized care plans, and regularly evaluate patient conditions for a panel of geriatric patients residing in long-term care facilities and private homes. Perform minor procedures and wound care as required, educate patients and families on health management, and coordinate with nurse practitioners and other healthcare professionals. Adjust treatment plans based on patient responses while utilizing a mobile-based EHR with AI functionality to reduce administrative time. On-call rotation is phone triage only with very low volume, typically one night per month. Full-time employment with Monday to Friday hours, no evenings or weekends, and a base salary ranging from $180,000 to $225,000 plus quarterly bonuses.
