Job Summary
Complete evaluations, reassessments, and discharge visits for patients in their homes across the New River Valley. Develop individualized plans of care based on patient goals, provide skilled physical therapy services, and educate caregivers on therapeutic exercises and home safety. Monitor progress to adjust treatment plans as needed while collaborating with physicians and the interdisciplinary team. Document all clinical activities accurately using WellSky. Maintain an active Virginia Physical Therapist License and hold a valid driver's license. Full-time employment with flexible daily scheduling, a manageable caseload, and $5,000 sign-on bonus available.
