Job Summary
Conduct comprehensive physical therapy evaluations to assess residents' mobility, strength, and functional capabilities, then develop and implement personalized treatment plans focused on pain management, improving mobility, and preventing falls. Administer hands-on therapeutic interventions including manual therapy techniques, therapeutic exercises, and balance training while utilizing and recommending appropriate assistive devices such as walkers or canes. Collaborate closely with nursing staff, occupational therapists, and caregivers to ensure a seamless continuum of care, and educate residents and families about home exercise programs and safety precautions. Maintain accurate and timely documentation in compliance with skilled nursing facility regulations. This full-time role serves a geriatric clientele recovering from surgeries or managing chronic illnesses at an outpatient office in Centreville, Virginia.
Required Qualifications
- Active and valid Physical Therapy license in Virginia
- A Doctor of Physical Therapy (DPT) or Bachelor's/Master's degree in Physical Therapy from an accredited program
Desired Qualifications
- At least two years of previous experience in a Skilled Nursing Facility (SNF) or geriatric rehabilitation setting, focusing on providing care that meets the unique needs of older adults
- Familiarity with long-term care regulations
- Knowledge of evidence-based rehabilitative practices
