Job Summary
Direct and manage physical therapy treatment plans to maximize patient capabilities, prevent disability, and alleviate pain. Conduct thorough evaluations using diagnostic assessments to determine functionality and formulate individualized care strategies. Deliver hands-on therapy including exercises, traction, and therapeutic techniques tailored to each client's condition. Supervise Physical Therapy Assistants to ensure regimens align with established care plans. Assess and recommend home environment modifications to facilitate rehabilitation. Collaborate in interdisciplinary team meetings to share progress and treatment modifications. Maintain accurate clinical records consistent with OASIS standards and provide education to clients and families regarding treatment plans.
Required Qualifications
- A Bachelor's degree in Physical Therapy from an accredited program
- Valid current PT licensure in good standing in New York
- Current CPR certification
- Valid driver's license with automobile insurance
- Exceptional critical thinking, decision-making, and interpersonal communication skills
- Ability to demonstrate creativity in treatment planning
- Commitment to leading innovative patient care initiatives
Desired Qualifications
- Doctorate in Physical Therapy
- At least one year of relevant experience in a physical therapy role
- Experience within acute care or home care settings
- Proficiency in using regulatory compliance knowledge, particularly federal, state, and governing agency guidelines
- A history of advocating for patients and families with emphasis on education and engagement in treatment processes
