Full-Time Physical Therapist - Home Care Services

Job Summary
Conduct comprehensive evaluations of patients with orthopedic conditions, post-surgical cases, and sports injuries to identify needs and goals. Develop and implement individualized treatment plans utilizing manual therapy and therapeutic exercises to facilitate healing and improve function. Educate patients on movement mechanics and home exercise protocols while documenting progress in the Electronic Medical Records (EMR) system. Collaborate with physicians and nursing staff to adjust care plans and manage complex cases involving elderly individuals with mobility limitations. Work onsite in patient homes Monday through Friday from 9:00 AM to 5:00 PM.
Required Qualifications
- A valid Physical Therapy license to practice in the state of Oregon
- A Bachelor's degree in Physical Therapy
- Strong communication skills to engage effectively with patients and their families
- Ability to create and execute evidence-based treatment plans tailored to individual patients' needs
Desired Qualifications
- A Doctor of Physical Therapy (DPT) degree
- A minimum of 2 years of clinical experience, particularly within a home care setting or assisted living facility
- Experience in managing complex cases involving multiple comorbities in elderly patients
- Demonstrated ability to work independently and collaboratively within a multidisciplinary team
- A passion for helping patients achieve their goals and improve their overall quality of life
