Job Summary
Conduct thorough assessments of residents to evaluate physical capabilities and identify tailored therapy goals. Devise and implement individualized treatment plans to restore mobility, enhance functionality, and alleviate discomfort, regularly reassessing based on patient progress. Provide hands-on interventions including therapeutic exercises, balance training, and mobility drills. Educate residents, families, and caregivers on effective mobility techniques and home exercise programs. Collaborate with a multidisciplinary team of nurses, occupational therapists, and administrative staff to ensure cohesive care. Maintain comprehensive documentation of treatment plans, metrics, and outcomes in alignment with SNF regulations. This role focuses on elderly residents and individuals with chronic conditions or post-surgical recovery needs within a modern, team-oriented Skilled Nursing Facility environment.
Required Qualifications
- A Doctorate or Master's degree in Physical Therapy from an accredited institution
- A current and valid Virginia Physical Therapy license in good standing
- Exceptional communication and interpersonal skills that facilitate a patient-centered approach to care
- Strong organizational skills with the ability to manage multiple patient schedules and documentation while ensuring quality patient care standards
Desired Qualifications
