Job Summary

Develop and implement individualized treatment plans based on comprehensive assessments of patients' physical abilities and conditions. Guide patients through therapeutic exercises and manual therapy techniques designed to increase mobility and alleviate pain. Educate patients and families on proper body mechanics, injury prevention, and home exercise programs to support recovery. Utilize various therapeutic modalities such as electrical stimulation, ultrasound, and heat/cold treatments, ensuring patient comfort and safety. Monitor and document patients' progress meticulously, tracking improvements in strength, functionality, and pain levels. Prepare treatment areas, ensuring all necessary equipment and supplies are organized and available for each session. Collaborate closely with a multidisciplinary team, including Occupational and Speech Therapists, to coordinate comprehensive care plans. Offer emotional and motivational support to patients throughout their rehabilitation process, fostering an encouraging and positive environment. Adhere strictly to all ethical guidelines and professional standards, maintaining patient confidentiality at all times. Schedule Monday to Friday from 8:00 am to 5:00 pm with occasional flexibility.

Required Qualifications

  • A Bachelor's Degree in Physical Therapy from an accredited institution
  • Valid state license or certification to practice as a Physical Therapist
  • Minimum of 1 year of relevant experience in a clinical or home care setting
  • Demonstrated ability to develop and implement effective rehabilitation strategies
  • Strong interpersonal and communication skills to effectively interact with patients and their families

Desired Qualifications

  • Experience in a home care environment, particularly with geriatric populations
  • Knowledge of a variety of rehabilitation methods and practices
  • Proficiency in using electronic health record (EHR) systems for documentation and patient tracking
  • Familiarity with interdisciplinary team collaboration to enhance patient care
