Job Summary
Conduct thorough evaluations of geriatric patients to create individualized treatment plans utilizing therapeutic exercises, manual therapy, and modalities. Monitor and document patient progress in EMR systems while adjusting treatments based on response. Collaborate with a multidisciplinary team including physicians and occupational therapists to provide comprehensive care. Educate patients and families on therapeutic techniques and implement fall prevention strategies tailored for the elderly population. Maintain up-to-date knowledge of evidence-based practices through continuing education and uphold ethical standards in all interactions.
Required Qualifications
- A graduate degree in Physical Therapy from an accredited institution
- Active state licensure as a Physical Therapist
- Strong interpersonal and communication skills
- Proficiency with electronic medical record systems and documentation methods
- A thorough knowledge of evidence-based practices that can be applied in clinical settings, particularly as they relate to geriatric therapy
- A commitment to patient-centered care
- Ability to work collaboratively within a multidisciplinary team
