Job Summary
Physical Therapist in Home Health evaluates the patient's functional status and needs, develops and implements a treatment plan to improve mobility and relieve pain, and treats patients to achieve maximum functioning. Responsibilities include conducting evaluations, providing therapy within scope of practice, coordinating with the health-care team, documenting progress notes, and effectively communicating the plan of care to patients and families. Required Qualifications
