Job Summary
Conduct comprehensive physical therapy evaluations to establish baseline metrics, then develop and implement individualized treatment plans using evidence-based practices. Reassess patient progress regularly to adapt interventions for orthopedic injuries, neurological disorders, post-operative recovery, and chronic pain conditions. Utilize the electronic health record system to maintain meticulous documentation, including SOAP notes and progress reports, while providing education to patients and families. Collaborate effectively with colleagues across disciplines to ensure integrated care and achieve minimum utilization metrics aligned with clinic business objectives.
Required Qualifications
- A Master's or Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program
- Current and unrestricted licensure to practice in Connecticut as mandated by the Board of Physical Therapy
- Willingness to provide services in person and via telehealth as clinically appropriate
- A genuine passion for working with a wide spectrum of patients across different ages, backgrounds, and conditions

Desired Qualifications
